Leyburn Medical Practice Patient Participation Group

Welcome to Leyburn Medical Practice Patient Participation Group which was officially formed at the end of July 2011 with the key purpose of acting as a link between the Practice and its patients. It is run by patients for patients and its specific activities are to:

  • Contribute to practice decision-making and act as a forum for consultation on service development and provision;

  • Provide feedback on patients’ needs, concerns and interests and challenge the practice constructively whenever necessary. This will include reviewing the Practice’s annual local patient survey in order to inform the Group’s priorities and work programme;

  • Serve as a ‘safety valve’ for dealing with grumbles and complaints about the practice – representing patients but also helping them to understand the practice’s viewpoint;

  • Communicate information about the wider community which may affect healthcare;

  • Give patients a voice in the organisation of their care;

  • Promote good health and higher levels of health literacy by encouraging and supporting activities within the practice and promoting preventive medicine;

  • Influence the provision of secondary healthcare and social care locally;

  • Monitor services, eg hospital discharge and support when back in the community; and

  • Give feedback to NHS trusts, commissioning bodies etc on consultations.

Annual Work Plan

The Committee has started to draft the 2012 Work Plan which will cover four main topics:

1 Group Recruitment and Demographic Representation - in order to mirror the Practice Patient List's demographics, we need to recruit more Group members from outside of Leyburn and from the under 40s age group.

2 Patient Transport - travelling to and from hospital and other clinic appointments is causing some patients real difficulties, especially when many now have to travel to Middlesbrough. Work will be carried out to understand the extent of the problem and to explore local options.

3 Monitoring Practice Performance - the Quality Outcomes Framework (QOF) is a measure of the Practice's achievement against nationally set quality targets. Work will be undertaken to enable publication of the Practice's QOF results.

4 Choice - patients are not always fully aware of the extent of choice that they have for hospital referrals. Work will be undertaken to establish the degree of patient awareness in terms of the choices that they have, and to consider how to provide with them better data to enable them to make informed choices.
